FOR the purpose of commending the local faith community, Sinai Hospital and LifeBridge Health, and the Maryland Citizens' Health Initiative Education Fund for their exciting new commitment to improving public health throughout Baltimore with the Maryland Faith Community Health Network.

Recitals
WHEREAS, On February 15, 2016, local faith congregations, LifeBridge Health facility Sinai Hospital, and the Maryland Citizens� Health Initiative Education Fund launched the Maryland Faith Community Health Network.
WHEREAS, The Maryland Faith Community Health Network brings together hospitals and faith congregations throughout Baltimore City to improve the health of the community.
WHEREAS, The Maryland Faith Community Health Network is based on a similar model in Memphis, Tennessee, which has been successfully improving health outcomes since 2007.
WHEREAS, The Maryland Faith Community Health Network welcomes other participants in Baltimore City.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF BALTIMORE, that the Baltimore City Council commends the local faith community, Sinai Hospital and LifeBridge Health, and the Maryland Citizens� Health Initiative Education Fund for their exciting new commitment to improving public health throughout Baltimore with the Maryland Faith Community Health Network.
AND BE IT FURTHER RESOLVED, That a copy of this Resolution be sent to the Mayor, the Health Commissioner, the President and CEO of LifeBridge Health and the Mayor�s legislative Liaison to the City Council.
